PHOENIX An overnight collision in Phoenix on Sunday left a 38-year-old man dead and two other adults injured, authorities said. Police responded to a collision call near 67th and Virginia avenues around 3: 15 a. m., according to the Phoenix Police Department. Upon arrival, officers found two vehicles involved in the crash. One of the drivers, Juan Andres Ortiz, was found with serious injuries. Life-saving efforts were made, but he was pronounced dead on scene. Two other adults were found injured from the crash and were taken to the hospital. The driver of the other vehicle, 19-year-old Andy Lepe, showed signs of impairment, police said. He was arrested and booked on multiple charges in connection with the incident, including manslaughter, aggravated assault and endangerment. More details on Phoenix crash An investigation indicated the collision occurred when Lepe was driving northbound on 67th Avenue when Ortiz’s vehicle made a left turn onto Virginia Avenue from southbound 67th Avenue. Lepe was allegedly driving at a high speed with no headlights on prior to the crash. The investigation remains ongoing.
